What happened
An employee was assembling a 12 ' ' valve body. The employee was putting the follower shaft in the valve and pushed the follower shaft in to far then rotated the ball to the open position by using a wrench on the drive shaft. When the employee was trying to work the follower shaft back to line it up properly the Taper Key fell out because it had not been driven in yet causing the v-ball to roll back to the close position catching their finger in between the ball and back edge of the valve.
