The employee was working on the bread line placing bread inside boxes. The bottom conveyor belt that transport the boxes had a piece of conveyor that was sticking out pass the side guard about 4 to 6 inches off the floor. The employee stepped near that area and their foot got caught with the slack of the blue conveyor belt causing the employee to trip and fall. A coworker broke the employees fall. The employee hit a nearby rack with their arm and shoulder and twisting their right ankle hitting.
