Before the incident
Put food in toaster oven at ~[REDACTED] working Reversed Forklift into staging with empty racks on lift all the way back close to break room Then got food pot pie which scorching hot put food on lap no seatbelt left hand on wheel right hand steadying the food. Thoughts was to drop off rack at producitoon then drive forklift outside to car and eat with girlfriend.
What happened
When started driving did not see pole becasue it was blocked by load and mast and it is verticle so. In general he was distracted because of plan and did not realize pole was there Speed was excessive as I was in a hurry to get food outside.
