Before the incident
EE was helping offload 36x130 2.9 green glass they strapped up the glass and noticed the front piece was broken they continued to try moving the glass from the A frame to the I rack.
What happened
The PIT driver told the EE 's to stand back and while moving it the forklift hit a bump causing some of the broken glass to fall. When thye got the load to the I rack they went to set it down on the rack and the EE was holding on to the strap and a broken piece that was still attatched dislodged and came down on the EE 's finger.
