Before the incident
The employee opened the retracting conveyor in bread wrap. When the retracting conveyor fully opened the employee engaged the handle to close the conveyor before proceeding to pass in between. They ended up pinching their arm between the slicer feed conveyor and the retracting conveyor. There was no need to close the conveyor before passing through it and there is no reason to place their arm between the two conveyors. The employee was using their arm that was pinched to hold the bread back so it did not fall on the floor. The employee was also not paying attention to what they were doing and did not realize their arm was caught in a pinch point.
