Before the incident

On [REDACTED] at [REDACTED] An ACP generalist was working in ACP Teachin She saw a red light which signals there is a fault in her zone She attempted to clear the fault by pressing the blue button She then stepped off the teach in platform and went around to zone 14 conveyor 48 She leaned over the conveyor to exam the zone and she what the fault was The zone was left in auto Since there was no active fault the pallet in the previous zone began to move When the pallet moved it caught her hair and the left side of her face was struck by the CHEP pallet She eventually broke free from the pallet and fell to the floor Another ACP team member saw her on the ground and had her go to the ACP control office
