Before the incident
On [REDACTED] at the time of [REDACTED] D2S day shift selector in training [REDACTED] who is in Week 15 of training was selecting in aisle 27 as he was selecting in the aisle around slot 55 several selectors were trying to drive pass him in the aisle so he adjusted the position of his MHE pallet jack to move to the left side of the aisle After doing so [REDACTED] then found himself next to slot 65 and needed to grab a case out of slot 55 [REDACTED] then started to reverse his MHE to get to his slot while reversing and looking into the direction he is reversing to the corner of his pallet struck the blue racking causing him to hit his mouth and tooth on the load rack bar
