What happened
On [REDACTED] An employee was using an air hose to blow out the Matrix baggers. The employee was using a portable stair case that was lined up square with the bagger. When the employee moved the ladder over to the right she rotated the ladder so that the stairs were no longer behind her but to her left. The employee slipped off of the left edge of the platform and fell down the two steps to the floor. The employee had severe pain and could not move her left elbow. [REDACTED] Ambulance responded to the scene and transported her to Wyoming County Community Hospital for evaluation. On [REDACTED] The employee brought in a work note keeping her out of work until [REDACTED]. The employee is also to follow up with a specialist.
