Per employee incident report: While at work on [REDACTED] I stretched my arms and immediately felt a sharp pull and pain in my back. I have a prior history of back problems due to bulging discs and this movement appeared to aggravate that condition. The following morning on [REDACTED] my back pain had worsened significantly. As a result of the pain and limited mobility I lost my balance and fell at home further injuring my back. I was subsequently hospitalized and treated for these injuries.
