Employee had just de-accessed a port and was placing gauze on the port

Per employee via incident report I accessed the pt 's port to give the pt a medication and de-accessed the port so she could be discharged home. I de-acessed the port and thought it had fully retracted and locked in place but the tip was still exposed. I had it in my hand as I was grabbing the gauze with my other hand to put on the port when the needle stuck in the the left hand under the left ring finger through my gloves. I dropped it as soon as it stuck me. I then picked it up and disposed of it in the sharps container. There was some minor bleeding from the site. I washed my hands with soap and water right away. I then notified charge and the supervisor and checked myself in to be seen. What conditions influenced the incident (weather equipment malfunctions time of day stress etc)? None other than I thought the needle was fully retracted and safety locked in place but the tip was still exposed

Bloodborne Pathogen Exposure Puncture Contaminated needlestick LEFT hand palm

Contaminated Port-a-Cath needle
