Per employee incident report: Walking between Daycare parking lot and crossing [REDACTED] [REDACTED] tripped and fell face-first on uneven payment in crosswalk and landed on right side of face lips chipped tooth (right side) abrased right facial cheek and fell on both knees but more on right knee. What conditions influenced the incident (weather equipment malfunctions time of day stress etc)? I believe the pavement in the crosswalk was uneven and caused me to catch my foot throwing me forward face-first.
