Associate states that on [REDACTED] she was pulling linen and felt something pulled in the upper portion of her back. She did not report it to her supervisor or nursing supervisor. She was previously scheduled OFF on [REDACTED] and took her own PTO day on [REDACTED]. [REDACTED] [REDACTED] she reported it to her manager [REDACTED] who directed her to report it to [REDACTED]. Incident report form filled out and scanned into OHM. Associate states that she feels completely fine today after 2 days of rest and does not want any more days off. She states she did not seek any medical treatment nor did she take any mind-altering drugs. Associate states she only took over the counter Acetaminophen from her own medicine cabinet at home. Associate states she currently is not taking any medication for her back and feels she is able to perform all her duties without restriction.
