What happened
[REDACTED] was being put back up. The employees had pre-threaded the line in prep to start up. The Lead Operator asked the IE (Material Handler) to feed the web through the pre-threaded line. To do so the web must be cut. The IE reached with their left hand and grabbed the web. At the same time a Packer employee assisting in the line startup process and in the same area utilized a utility knife to cut the web without recognizing the Material Handlers left hand.
