What happened
On [REDACTED] at approximately [REDACTED] an employee was involved in a minor forklift collision while traveling from [REDACTED] toward the docks without carrying a pallet. As the employee approached [REDACTED] the forklift was abruptly stopped to avoid another forklift passing in front. In an attempt to prevent a collision with the other forklift’s forks the employee quickly shifted into reverse without first looking behind. As a result the counterweight of the forklift made contact with the counterweight of the other forklift. The collision was minor with no visible damage or major impact reported. An injury report was completed as part of standard procedure. The employee declined medical evaluation and refused treatment at the time of the report.
