What happened

[REDACTED] had just came out of the office looking for a coworker. A [REDACTED] after she walked out I heard a loud noise and a grunt. I stepped out the office and saw [REDACTED] in front of the paper shredder on her buttocks leaning toward the left with her legs bent under her. She said this damn wax. She was offered assistance to get up and declined it. She was able to get up on her own and ice packs were offered and accepted. The floors in varying sections were completed through out the night not sure of the solution but it was slippery. There was a wet floor sign near [REDACTED]. Stated she will be going to Concentra for follow up after shift.
