Before the incident
Pt was attempting to exit through the emergency exit door by room 511 and PSA was holding the door shut to ensure that the pt did not exit. Patient then started cursing scratching biting hitting and punching the PSA. PSA called for help.

The patient scratched the PSAs face leaving visible markings and PSA yelled help where other staff members proceeded to get the patient away from the PSA.
