Pulling a gate pole out of a cheek.
What happened
The employee was tied off on top of a mold. When he pulled the gate pole out of the mold it was top heavy and started to fall forward. This caused him to lose balance and fall as well. The harness and SRL caught him and caused him to swing hitting his back on the mold.
