climbing over the pipes to adjust pipe wrench

was loosing up a valve to unclog a screen was climbing on valve pipe system for better leverage with wrench after adjusting and fixing issue he then proceeded to climb down and his foot slip on the pipe which caused him to loose his footing and fall which resulted in him being impaled by the valve handle in his left inner thigh was then transported by ambulance to WCH emergency department
