On [REDACTED] at around [REDACTED] [REDACTED] was putting away a scooter into a gaylord using PPE (gloves) but did not request a team lift. As he placed the scooter into the pallet it became stuck. In an attempt to restack the scooter [REDACTED] tried to adjust the bike next to it by its wheels. During this process his right hand slipped and got caught in the bike 's chain. [REDACTED] managed to free his hand and immediately reported the incident to me at [REDACTED]. He sustained a small puncture injury to his right pinky which was not bleeding. At the time [REDACTED] did not want medical attention only first aid and requested to return to work. I provided first aid and new gloves. On [REDACTED] after checking in with [REDACTED] he shared that his pinky was progressively hurting and he wanted to seek medical attention. We then called the emergency hotline to begin his claim process.

While attempting to pull a bike to the side with the back tire his fingers slid and got caught into the bike chain.
