Before the incident
[REDACTED] at approximately [REDACTED] [REDACTED] was loading his final Tv 's into the trailer at dock door 345. While trying to maneuver tvs around the truck [REDACTED] was down stacking a tv while pulling it down his hand got struck smashed between the clamp paddle of the forklift and the tv he was trying to move. This incident caused swelling in the right hand coin sized laceration. He was directed to seek medical attention.
