At approximately [REDACTED] on [REDACTED] [REDACTED] was shaking a box of [REDACTED] out of the outer case when one fell onto her left foot. She had no immediate pain and did not report it. She began feeling pain the following week (week of [REDACTED]). She sought medical treatment for her pain and was taken off work and was given a boot for her foot. She returned to work on [REDACTED] but left early due to being in pain. Her injury was reported to me by other team members so I reached out to [REDACTED] on [REDACTED] to see if the injury occurred at work. She confirmed that it occurred at work but said she did not report it as she was not in pain at first did not want to file a worker?s comp claim.
