At approximately [REDACTED] [REDACTED] had his left foot on the track of the extendable conveyor in [REDACTED]. Another team member was unaware [REDACTED]?s foot was on the track and began retracting the extendable. One of the wheels of the extendable ran over [REDACTED]?s foot. [REDACTED] reported the situation to his leader immediately. [REDACTED] was given first aid on site for bruising and bleeding on his left foot. He did leave work to seek medical attention.
