Before the incident
On [REDACTED] a Radiologic Evaluator was inspecting part number 30905 using a read fixture. During the process the employee moved to the opposite side of the fixture to partially relieve the threaded screw braking device on the backside rollers. Upon returning to the inspection side the employee’s left middle finger was crushed between the roller and the flange of the part while rotating it. The employee was wearing the designated PPE for handling castings and performing a routine task. The incident was immediately reported to the shift lead and another employee provided first aid. The department supervisor and safety personnel contacted AMR and the employee was transported to the hospital for treatment. All 30905 fixtures were placed on Stop Work until improve guarding or handling methods are developed.
