Before the incident

On [REDACTED] as I was doing my morning walks [REDACTED] stated that she felt some discomfort in her upper back I asked her if she needed immediate medical attention and she stated no I asked [REDACTED] what had happened and she stated she wasn 't sure but on [REDACTED] she was lifting up two totes at a time and felt fine throughout the day but when she woke up on [REDACTED] she felt a little sore tired from her back [REDACTED] stated that it wasn 't a pain but just felt tightness and thought it was soreness from the days work from the previous day. [REDACTED] stated that it would feel better over the weekend when she rested but feels the same tightness still and just wanted to report it. I let her know that we would call the nurse hotline and go from there. The nurse recommended [REDACTED] see a doctor within three days. [REDACTED] stated she could finish the day as the nurse did not send her home. I asked her how she felt and stated she felt fine but just the tightness in her back. I
