Delivering a dewar to a customer

There was a faulty liquid valve and the absence of a dump valve on the customer’s Dewar. The employee sustained 1st degree burns on his hands and was able to return to work with minimal restrictions. To fully understand and confirm what happened our team performed testing. Our tests showed the Dewar retained pressure even when vented requiring manual hose checks and creating safety risks. The team identified the missing dump valve as the root cause and discussed new safety procedures including verifying zero pressure before disconnection and mandating LPG-style gloves for protection from cold exposure and liquid leaks
