WHILE WORKING THE EMPLOYEE EXPERIENCED A SUDDEN ONSET OF BLURRED VISION DIZZINESS AND SEVERE HEAD PAIN. DUE TO THE SEVERITY OF THE SYMPTOMS THE EMPLOYEE CONTACTED [REDACTED] AND REQUESTED MEDICAL ASSISTANCE. MEDICAL TREATMENT PROVIDED: THE EMPLOYEE WAS EVALUATED ON SCENE BY MESA FIRE & MEDICAL AND WAS FOUND TO BE CONSCIOUS AND ALERT. THE EMPLOYEE WAS TRANSPORTED BY AMBULANCE TO MERCY GILBERT HOSPITAL FOR FURTHER MEDICAL EVALUATION.
