Before the incident
At [REDACTED] in the Ti Cleaning Department’s Hotsy pit on the Protex Wash side a Shell Removal Operator sustained an eye injury from ricocheted spray and debris while removing a chain sling after washing a part. The operator had raised the face shield and was wearing only safety glasses instead of the required tight-fitting goggles and face shield. The safety door divider was in place but the operator was exposed to debris from another worker performing gross shell removal on the opposite side. Post-incident interviews of several operators cited fogging issues with goggles and gasketed safety glasses can be an issue especially with the chemical and cut-resistant gloves attached to the chemical suits. It was also noted the injured operator did not use any of the four available eyewash stations within the first 5-6 minutes and instead appeared to leave department to rinse eye in a restroom.
