What happened
The trench constructed of soldier pile and lagging where the soldier piles had a top 12x12 timber brace. The pipe being worked on was set in the trench a [REDACTED] and was being adjusted for fit-up. This pipe was rigged to a crane with 2 endless slings in a 2-way configuration. It appears one of the slings came in contact with 12x12 timber brace at the top of the soldier piles causing it to slide off of the soldier pile striking the IE on the lip.
