What happened
Employee was walking in area they were unauthorized to be in. Employee states that they were talking to another forklift driver that was walking with them and was not paying attention to the PNF alarms and lights that were going off for a rack that was coming down the drop. The other forklift driver shouted for her to watch out and attempted to move the rack but she had already made forehead-first collision with the rack. She insisted on just a cleanout and that she did not want to go to the doctor repeatedly then decided after she went home that she wanted to go tot he ER where she received 4 stitches and an irrigation of the wound.
