What happened
To reach the top of the panel and secure the rigging equipment the employee leaned a 6-foot straight ladder against the box and stood on the second rung. While still on the ladder the employee instructed the operator of the excavator ([REDACTED]) to apply tension to the rigging. As the tension was applied the box shook slightly causing the ladder to slide. This led to the employee falling approximately 2 feet to the ground resulting in pain in the left ankle.
