The employee was assigned to set and secure wall forms during panel installation. This work required the employee to be positioned above the forms to disconnect crane chains and install tighten form ties and wing nuts while the crew below was aligning the form.

While the employee was moving between the forms and holding onto the waler for support; the crew below adjusted the form to align it. During this movement; a form tie loosened slid; and the wing nut pinched the employee finger between the waler and the wing nut; resulting in a finger injury.

The employee sustained a crush injury to the finger on the left hand when it became pinched between a form tie wing nut and the waler as the form was being adjusted for alignment.

A form tie and wing nut; which shifted during form alignment and pinched the employee finger against the waler.

Left finger: Pinched by form hardware during adjustment.
