The employee was assigned to assist with crane operations; including guiding the crane operator and hooking and unhooking deadman blocks as materials were being flown into place. The work required the employee to walk repeatedly across the jobsite; which consisted of uneven; rocky ground. The area had been previously worked by another contractor; and exposed rebar remained in the ground.

After unhooking a deadman block; the employee was walking back to hook up the next load when he lost his footing on the uneven ground and fell forward. During the fall; the employee knee struck an exposed piece of rebar; causing a puncture cut injury to the knee. The incident was immediately reported; and the employee was taken for medical treatment.

The employee sustained an impalement puncture (gash) injury to the right lower knee area after slipping and falling and striking an exposed piece of rebar.
