Before the incident
Chipping over pour to make way for floor supports.
What happened
While removing excess (over pour) concrete between floor levels to install floor supports a section of the floor slab which was secured to metal studs and an angle plate detached and fell approximately 15 to 20 feet to the lower level of the scaffolding. During the fall the concrete slab and angle plate (estimated combined weight (150–250 lbs) struck a team member on their left shoulder impacting their left thumb knocking the hammer drill from his hands.
