What happened
Employee states: He was trying to push pull a piece of wood out of the way of the chain and the tip of the glove caught the chain pulling the hand inside (the point of operation chain and sprocket) Surveillance footage (IP Camera 08): Footage shows [REDACTED] feeding raw material (14 feet sections of lumber) into the KM-16 as the accident occurs. It is difficult to see exactly where the injured employees right hand and arm (injured) is placed moments before the accident occurred. Witness [REDACTED] is standing alongside of [REDACTED] sorting the pieces of wood that are being cut and stacking sorting them beside the conveyor. As the incident occurs the footage is somewhat blocked by the pallet of stacked cut boards the conveyor and by [REDACTED] (where he is standing) Surveillance footage (IP Camera 15): Footage shows [REDACTED] walking over to the KM-16 to help as he is being asked by [REDACTED] to help with the situation.
