What happened
The expanded needed wiped down due to lubricant traveling down the mandrel. . When he went to clean off the lubricant from the expander he did not notice that a roll pin that holds the chuck on the roll machine came out about a 1 4 this caught the rag then pulled his left hand into the tube expander.
