Before the incident
Packing [REDACTED] and was using a box cutter to open a box and cut fer left index finger @ approx [REDACTED] then cut her left pinky finger @ approx [REDACTED]. After cleaning and applying bandages our [REDACTED] as well as those working near her said she seemed really off. I had the Safety Officer drive her to Mercy Occ for an alcohol test and they did 2 within 15 minutes of each other. The results of the BAC were .200 and .205.
