Before the incident
I was notified on [REDACTED] by [REDACTED] along with a trasnlater that on [REDACTED] at approx [REDACTED] he walked in to a pallet in the warehouse and hurt his right foot. I had our Safety Officer fill out a Report of Injury and I gave him a Card with the name address and W C info to take with him to Mercy Occupational. He refused and said he just wanted to go home and put his foot up. If it wasn 't better by [REDACTED] he would go. Instead he went to [REDACTED] and texted his Manager which said he could return on [REDACTED]. He continued to text his manager pictures of his foot and didn 't go to Mercy Occ until [REDACTED] and was released to regular duty on [REDACTED].

Walking through the warehouse. There were no witnesses and I couldn 't find this incident on the cameras for that date and time.

Cutaneous abscess of right foot. Nondisplaced fracture of fourth metatarsal bone right foot initial encounter for closed fracture.
