What happened
Two operators were adjusting the bail on the Bradley die lifting rig. The first operator was using an air wrench on the adjustment screw while the second operator was marking the stopping point with his right middle finger. When the first operator activated the air wrench the bail moved rapidly in the wrong direction pinching the second operator’s right middle finger between the bail and the end-bail stop on the east side of the rigging. This resulted in a laceration to the distal portion of his right middle finger.
