The operator had just finished making adjustments to corrugate body wraps at infeed conveyor of Moen Box [REDACTED] 3-2.

The operator was stepping down (approximately 17 inches above the production floor) onto a lower step. Her left heel stepped on the edge as opposed to the lower step. This caused the operator to lose balance and fall forward.

Operator sustained a fractured left wrist and sprained left ankle.

Impact with concrete production floor.
