What happened
the employee ignited the cutting torch to attempt to clean a broken welded area on a piece of machinery. When he started to use the torch he noticed the flame was not coming out properly at the tip of the torch and looked down at the torch. The ignited tip of the torch caused flammable gas escaping out of an unidentified defective section of the torch to suddenly ignite and the employee inhaled some of the ignited vapor.
