The employee was boxing products on a conveyor as a temp worker in the Value Added production packing area.

The employee had finished boxing a product when they reached over a conveyor and under another conveyor next to it to pick up a product that was on the floor. This put her head at conveyor height. The conveyor is a mobile conveyor with locked wheels however it wiggles about 1-2 when other employees push pack product along on it. As [REDACTED] had her head at the conveyor it moved 2 and came into contact [REDACTED] 's forehead while she was bent over.
