Testing Hydraulic cylinders

An employee needed access to a hydraulic cylinder located on a rolling rack which was blocked by two other racks. Instead of repositioning the racks to safely retrieve the cylinder the employee chose to manually lift and carry the 138-pound cylinder to a nearby empty rack. While attempting to place the cylinder onto the new rack it rolled away causing the employee to lose balance and fall forward. The momentum of the cylinder contributed to the fall resulting in the employee striking their forehead on the bottom of the rack and sustaining a significant laceration. The employee also injured their thumb while trying to brace the fall.
