Before the incident
Crew was stripping a column form from the west face of a column. The employee was serving as the signal person for the crane operation
What happened
When the column form broke loose the signal person directed the crane operator to cable up. As the form was being lifted it began to rotate and the bottom horizontal strongback became caught under the top of the south face column forms horizontal strongback. The strongback turned and ripped off the form striking the signal person on the head and back.
