Employee was walking across a rebar mat to retrieve 2x4 lumber from the other side of the pour area.

While traversing the rebar grid the employee 's leg fell through an opening in the rebar mat. The rebar mat had no wire mesh or plywood walkways in place. The employee lost his balance and fell forward resulting in leg fractures.

Two fractures to the left tibia and fibula; required surgery.
