The column crew was disengaging rigging from a set column form. A rigger had directed the crane operator to cable down and one of the four-way hooks became caught on top of the rebar within the column

The employee attempted to free the stuck crane hook by pulling on the column strap. As the hook became unstuck it suddenly released and swung downward with force striking the employee 's hard hat and dislodging it. The impact struck the employee 's forehead causing him to fall to the ground. He was observed to be unconscious and exhibiting convulsive movements. Emergency medical services were immediately contacted.

Depressed skull fracture requiring surgical alignment; laceration to forehead requiring stitches; loss of consciousness; bruising and swelling to forehead and left eye. No traumatic brain injury identified.
