Before the incident
Employee was assisting with cleanup after a concrete pour. A foreman had discovered a trash can with chains and hooks buried inside and asked the employee to help remove them.
What happened
The employee was pulling on a chain that was entangled under trash and materials inside a dumpster. As he pulled the chain suddenly released from the debris and the chain hook flew upward striking him in the face and causing a deep laceration to his upper lip.
