What happened
The employee attempted to connect a two inch flexible hose to the bottom of a 350 gallon stainless steel portable tank. While making the connection he failed to notice that there was no isolating butterfyl valve attached to the discharge piping of the portable tank. Consequently two gallons of orange colored concentrate splashed onto his lower arms hands and lower extremities.
