**This is Precautionary *** Driver was trying to preform a delivery at United Refrigeration [REDACTED] [REDACTED] when moving the pallet from the nose to the rear the pallet crushed. the crushed pallet cause the pallet jack to get stuck underneath. when [REDACTED] forced it out he twisted and turned to pull it out. in the process he felt a sharp pain in the center of his back that got worst as the day progressed.
