Removing Cutter Head from wooden box
What happened
The Cutter Heads at the Moulder Department are placed inside a wooden box to transport the Cutter Head to and from the moulder machine and the moulder maintenance shop. An employee was flipping the Cutter Head from the wooden box onto the workbench. The employee placed his right hand beneath the box opening to guide the Cutter Head to the work bench. The Cutter Head slid out of the box and impacted the bottom of the employee 's hand while one of the knives punctured the employee 's bottom right middle finger.
