Before the incident

Mr. [REDACTED] was assembling on roof Table Six. Mr. [REDACTED] violated safety regulations by walking into the marked off area that all employees must stay clear of while the gantry is in motion. Mr. [REDACTED] is aware of his mistake and takes full responsibility.

What happened

While assembling on table 6 Roof Mr. [REDACTED] was pinned between the gantry and a support pillar. Mr. [REDACTED] actions are to blame because he intentionally stepped in front of a moving gantry and in doing so he was pinned between a portion of the gantry and a support pillar of the infrastructure of the plant. This caused an abrasion on Mr. [REDACTED] left shoulder blade. Paramedics were called and after speaking to them his only complaint was he got the breath knocked out of him and the abrasion on his back. The paramedics checked him over and asked him if he wanted to go to the hospital. Mr. [REDACTED] stated that he only wanted to get an x-ray to check for broken bones. The Paramedics said we did an amazing job prepping him for their arrival. He was taken to the NOVA Clinic and was seen by medical professionals and after an Xray was told there were no injuries except for the abrasion on his shoulder blade. No light duty. He said he feels fine and has no other issues to report.
